A new operation is described which allows the Fontan procedure to be used
in patients having single ventricle accompanied by aortic outflow
obstruction. Connection of the proximal pulmonary artery and valve to the
aorta via a tubular prosthesis provides a second outlet of the ventricle to
the aorta in order to effectively bypass subaortic obstruction. Anastomosis
of a distal pulmonary artery to the right atrium and closure of the atrial
septum by patch (Fontan procedure) complete the establishment of
unrestricted pulmonary and systemic circulations in series.
